November 12th - November 30th, 2024

This Open Enrollment period is your opportunity to make changes to your 2025 COBRA Continuation Coverage benefit elections. Please carefully review the important benefit and rate information on this webpage. A PDF viewer is required to view the documents posted.

If you do not wish to make any benefit changes, you do not need to do anything.
Your COBRA benefits will remain unchanged if BCC does not receive a 2025 COBRA Enrollment Form. New 2025 plan premiums outlined in the 2025 COBRA Rate Sheet are effective January 1, 2025.

If you wish to make changes to your 2025 COBRA benefits:
1. Download, complete, and sign the 2025 COBRA Enrollment Form.

2. Submit the form to BCC by mail no later than November 30th:

3. BCC will process your changes and new 2025 plan premiums will be effective January 1, 2025.

Enrollment Forms must be postmarked by November 30, 2024 to be processed.
